All students athletes 7 – 12th grade are required to fill out and submit these forms prior to each school year. Pasadena ISD and the UIL require the parent/legal guardian and student to complete the participation forms annually.
The below required participation forms must be completed in their entirety and returned to the athlete’s athletic trainer before a student participates in any tryout, practice, scrimmage, performance, contest or game before, during or after school. It is NOT necessary to create an account in Rank One. You can access RankOne at www.rankonesport.com and click on Parents/Students Fill Out Electronic Forms in the top center to get started.
